Understanding Suture Needle Types

Choosing the right suture needle is just as important as choosing the right suture material. The needle determines how the suture passes through tissue — affecting wound closure quality, tissue trauma, and ease of use. Here is a practical guide to the needle types you will encounter when ordering sutures.

Reverse Cutting Needles

Shape: Triangular cross-section with the cutting edge on the outer curve of the needle.

Why it matters: The cutting edge faces away from the wound, reducing the risk of the suture pulling through the tissue. This is the most common needle type in dentistry and dermatology.

Common needle codes:

  • P-3 (13mm, 3/8 circle) — The most widely used dental and dermatology needle. Found on most 5-0 and 4-0 sutures including Ethicon 687G (chromic gut), MCP494G (Monocryl), 8695G (Prolene), and VCP493G (Vicryl).
  • P-1 (11mm, 3/8 circle) — A smaller reverse cutting needle for fine tissue work. Common on 6-0 sutures like Ethicon 8697G (Prolene) and 1657G (plain gut).
  • FS-2 (19mm, 3/8 circle) — A larger reverse cutting needle for tissue that requires bigger bites. Used on 3-0 and 4-0 sutures like Ethicon 636H (chromic gut 3 doz) and VCP392H (Vicryl).
  • FS-1 (24mm, 3/8 circle) — The largest common reverse cutting needle. Found on veterinary sutures like Demetech PM942 (Monopro 3-0) and PM943 (Monopro 2-0).
  • PS-2 (19mm, 3/8 circle) — Precision reverse cutting for plastic and dermatologic surgery. Found on Ethicon MCP496G (Monocryl) and VCP497G (Vicryl).
  • PS-3 (16mm, 3/8 circle) — Precision reverse cutting for fine skin closure. Found on some Prolene and Ethilon products.
  • C-22 (20mm, 3/8 circle) — The standard reverse cutting needle for PTFE sutures in dental implant surgery. Found on Corza 817B.
  • C-6 (19mm, 3/8 circle) — Reverse cutting needle used on many Corza/LOOK sutures including chromic gut 559B and 558B.

Best for: Skin, oral mucosa, dental tissue, dermatology, cosmetic surgery.

Taper Point Needles

Shape: Round cross-section that tapers to a sharp point.

Why it matters: Taper needles push tissue apart rather than cutting it, creating a smaller hole and causing less tissue trauma. Ideal for soft, easily penetrated tissue like fascia, muscle, bowel, and blood vessels.

Common needle codes:

  • SH (26mm, 1/2 circle) — A standard taper point needle for general surgery. Found on Ethicon G122H (chromic gut 3-0) and Demetech PM316 (Monopro 3-0).
  • RB-1 (17mm, 1/2 circle) — A smaller taper point for fine tissue. Found on Demetech PM304 (Monopro 4-0) and Ethicon Y303H (Monocryl 5-0).
  • CT-1 (36mm, 1/2 circle) — A large taper point for thick tissue. Found on Demetech PD340 (Doxipro size 0).
  • CT-2 (26mm, 1/2 circle) — Medium taper point. Found on Demetech PD333 (Doxipro 2-0) and PD332 (Doxipro 3-0).

Best for: Internal organ tissue, fascia, muscle, blood vessels, veterinary soft tissue surgery.

Conventional Cutting Needles

Shape: Triangular cross-section with the cutting edge on the inner curve.

Why it matters: The cutting edge faces toward the wound, which increases the risk of suture pull-through. Largely replaced by reverse cutting in modern practice, but still found on some specialized products.

Common needle codes:

  • PC-1 (13mm, 3/8 circle) — Found on Ethicon 1915G (fast absorbing plain gut 5-0), one of the most popular dental sutures.
  • PC-3 (16mm, 3/8 circle) — Found on Ethicon 8636G (Prolene 6-0).
  • PC-31 (19mm, 3/8 circle) — Found on Corza 821B (PTFE 4-0).
